These prices are effective as of April 16, 2018 and subject to change without notice.
The goods and services shown below are those we can provide to our customers. You may choose only the items that you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.

Payment Policy
We understand that losing someone you love is a devastating experience. It can be very traumatic and seriously impact one's judgment. Therefore, it is important to be realistic with respect to funeral expenditures.
One of the most important responsibilities that we have to each family we serve is to bring your emotional and financial needs together, whatever the circumstances may be. We do not want your choices to be more than is affordable, nor less than you would like to arrange.
We understand the importance of a meaningful service and are here to assist you in any way we can. As part of our consultation with your family, we will discuss the different payment options available. Our goal is to insure the services and merchandise you select will be in keeping with your wishes, as well as your budget.
It is our goal to provide your family the finest in services and merchandise at a fair price. We offer a variety of options to fulfill payment.
All services must have financial arrangements for payment before any public announcement of final arrangements can be made.
Because of financial truth in lending laws, we are unable to provide financing or monthly payment arrangements through our services. You may want to contact your own financial institution to provide options if one of these options does not suit your needs. We encourage frank discussion of these matters, we will attempt to assist you in any way possible.
You may choose one or a combination of the following:
- Payment with cash, check, or credit/debit card at the time of arrangements.
- Payment with Life Insurance Assignment – proceeds from valid life insurance policies. Our acceptance of this mode of payment is dependent on the cooperation of the Beneficiary and insurance company. We must have complete cooperation in providing information on the beneficiary, amount of the death benefit available, outstanding loans information, cooperation getting claim forms completed in a timely manner.
- Note: Due to extremely long processing times and costs to obtain payment we have enlisted the services of a third party firm to process our insurance claims. Their fee to process the claim is 5% of the contract amount. This amount will be added to each contract where the family chooses to pay with a life insurance assignment.

We also realize that making arrangements in a familiar and more comfortable setting can alleviate this added stress. For that reason, we find it best to come to you to offer our service and merchandise choices in the security of your own home or other place of your choice.
Our goal is to provide you with every option of personalization in final care as an alternative to the conventional way funeral and cremation services have been.
We also believe that making arrangements in a familiar comfortable surrounding allows for clearer and wiser decision-making.
We come to you when you need us the most, offering services and products without you ever having to leave the comfort of your own living room.
Our personalized offerings include the coordination of services at this most difficult time. Assisting you with arrangements, visitation and funeral services scheduled through your house of worship or one in your area that desires to minister to you and your family.
Our current operations are carried out by using a contract crematory and embalming facility so that the savings we realize can be passed on to each family we serve.

Our services include: conducting the arrangements conference; planning of the funeral; consulting with family and clergy; shelter of remains; preparing and filing necessary notices; obtaining necessary authorizations and permits; coordinating with the cemetery, crematory, or other third parties. In addition, this fee includes a proportionate share of our basic overhead costs. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.)

Embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as a direct cremation or immediate burial.

This charge includes applying cosmetics, dressing and placing your loved one in a casket or alternative container prior to burial or cremation. This charge may also apply when there is a need to address any conditions when your loved one is received from another funeral home. The charge for a personal hairdresser or barber is NOT included in this fee.

Exact Charge
Transportation charges are included for locations within 40-mile radius from the funeral establishment. If we travel beyond 40 miles, we will charge an additional $2.50 per mile (one way). If an overnight stay is required due to no fault of ours, or our driver and vehicle cannot safely return the same day, there will be an additional charge of $175.00 per night (for motel and meal expenses) added to the automotive equipment charges.

Our charge includes: basic services of funeral director & staff; a proportionate share of overhead costs; transfer of remains to funeral establishment; and embalming. This charge DOES NOT include any additional professional staff services, use of staff services for viewing or visitation, a funeral ceremony or graveside service, transfer to an airport, shipping charges or merchandise. A combination shipping container ($350.00 for non-casketed remains), or a casket air tray ($150.00 for casketed remains) is required for airline transfer.

This charge DOES NOT include any additional professional staff services, use of the equipment and staff for viewing or visitation, a funeral ceremony or graveside service, transfer from the airport to the funeral establishment or merchandise.

Our charge for a direct cremation (without ceremony) includes: basic services of funeral director & staff; a proportionate share of overhead costs; transfer of remains; transportation to crematory; necessary authorizations; and cremation. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are fiberboard or cardboard.

Service Offerings
A funeral service should be unique, personal, and planned to meet the individual needs of each family. We feel that it is important to discuss all of the service options available with our families to allow them to make their own informed decisions. To this end, we have created several funeral service offerings to help you make those decisions.
A traditional funeral service acting in conjunction with a customary visitation is our specialty. Eric Thomas Funeral Service Licensee LLC will work hard to guarantee that all your needs are met if not exceeded. We will schedule the visitation at a time most convenient for you, family and friends, and if you would like to hold a visitation both the evening before the service and immediately prior, we can happily accommodate your desires. The funeral service can be held at a location or church of your choosing. Afterward, we will coordinate a procession so that friends and family can accompany you to the cemetery for a brief graveside service. The staff at Eric Thomas Funeral Service Licensee LLC understands the difficulties associated with losing a loved one, and we will use all of our combined experience and dedication to help you through this trying time.

State or local law does not require that you buy a container to surround the casket in the grave. However, many cemeteries require that you have such a container so that the grave will not sink in. Either a grave liner or burial vault will satisfy those requirements.
